My exim logs had:

... temporarily rejected RCPT : error in redirect
data: missing or malformed local part (expected word or "<") in
"fail: Account c losed due to spam; use alternative address."

This was caused because my exim alias was missing the first colon. Here it is with first colon (so three total):

account:	  :fail: Account closed due to spam; use alternative address.
